Authorized Agent Agreement (Template)
Last updated: Version 1.0 — July 1, 2026
Designation of authorized agent
The subscriber ('Principal') designates Brainstormen ('Agent') as their authorized agent for the purpose of submitting requests to access, opt out of the sale or sharing of, and delete the Principal's personal information held by data brokers, people-search websites, marketing data companies, and similar entities.
This designation is made under applicable privacy laws, including the EU General Data Protection Regulation (GDPR), the California Consumer Privacy Act as amended (CCPA/CPRA), and equivalent state and national privacy regulations.
Scope of authorization
The Agent may disclose the personal information provided by the Principal — legal and previous names, current and past addresses, email addresses, phone numbers, and date of birth — to covered entities solely as necessary to identify the Principal's records and effectuate their removal.
The Agent may sign and submit verification documents, follow up on non-compliant requests, and re-submit requests on a recurring basis for the duration of the Principal's subscription.
Term and revocation
This authorization remains in effect while the Principal maintains an active subscription and may be revoked at any time from account settings or by written notice, after which the Agent will cease submitting new requests.
Execution
The agreement is executed electronically during onboarding by the Principal typing their full legal name, which constitutes a valid electronic signature under the E-SIGN Act and eIDAS.
